We work with several U.S. health insurance plans and can submit claims directly for you. To make your visit easier, please bring the correct claim form and your completed New Patient Packet to your appointment. We know that paperwork can feel a bit overwhelming, but completing these forms in advance helps everything run smoother at your appointment so we can see you faster and avoid unnecessary waiting time. Please note that there are also insurance plans we do not have a contract with, so some patients may need to use different forms or pay upfront and seek reimbursement from their insurance later.
